Step 2: Complete the online registration in FinalForms (Opens April 1, 2025).

•  If you already have a child who participated in athletics in 24/25 at Washington HS or Washington MS, it is likely that you have an existing FinalForms account, click HERE to log in. DO NOT select “New Account”. “Click to Switch To 2025-2026 Registration” (If you forget your password, please follow the steps to reset it.)
•  FinalForms is our new student registration portal (we are no longer using OneView).  All parents/guardians enrolling a student must have a FinalForms account. Click HERE to create a FinalForms account. Select the blue 'new account' button. You must have an email address to create an account to register your child. “Click To Switch To 2025-2026 Registration”. Need help? Step 3: Attend the in-person registration for both you and your child on April 28, 2025.

• The in-person kindergarten registration will be held on April 28, 2025 from 3:30 PM – 6:00 PM in the cafeteria of Cherry Hill Primary School, 720 Oakland Ave.
• Your child will participate in a short assessment during the in-person registration.
• Please have all documents uploaded in FinalForms or the registration process will not proceed.
  • certified copy of birth certificate
  • parent driver license, state ID or current passport
  • student’s immunization records
  • parent’s proof of residency (must be one of the following)
    • deed/building permit
    • rental agreement
    • tax statement
    • voter registration card
    • mortgage/lease agreement
    • rent payment receipt/copy of money order for rent
    •  current utility bill
    • parent affidavits (forms 5111F2a/5111F2b)
    • current pay stub
    • letter from parent's employer on company letterhead to establish residency
  • student’s custody document (if applicable) Must be court file-stamped document or divorce decree (a notarized document is not acceptable)
  • student’s IEP/ETR (if applicable)
